Medication-related quality of life scores visit 1-2 score,Health-related quality of life scores were assessed from the Thai version of the KDQOL-SF questionnaire specifically for kidney disease visit 1-2 score
Participants in the Pharmaceutical care using PROMPT questionnaires group received the patient-centered principle by assessing the patients drug-related needs and finding problems with medication use (drug-related problems) of patients from the PROMPT questionnaire together with the medication use interview data, Patient medical records in HOSxp, patient identification book, and patient laboratory results and then evaluate the medication-related quality of life and health-related quality of life. ,Participants in the routine hospital standard services group received the services following hospital guidelines by the dispensary pharmacist after the doctors met and then evaluated the medication-related quality of life and health-related quality of life.
