Complete undated NHSRC submission checklist, including all proposal contents, attachments, visible bracket boxes, fee-receipt/funding items, missing-item explanation, signature/name/date and footer. Original U$150 or K5000 for Malawian Students and the 10% budget contribution are retained, without reconciling them with the different student/general fees in the later PHIM-linked Form 101 PDF or the 2026 regulations. The final source email ends at gmail as printed. This exact original is retained in the PHIM public media collection under a 2024 upload path. The upload path is not an issuance or effective date.
